Position Overview:

Works independently and in consultation with physicians and other health care providers across the health care continuum to ensure appropriate medication therapy for individual patients. Uses patient interviews and counseling, reviews the medical record and medication profile, consults with physicians, teaching, staff education, medication use evaluation, adverse reaction monitoring and resolves medication-related problems. Interventions target high risk population through the provision of medication reconciliation and comprehensive medication use management in focused disease states. Assist with care coordination, disease state education and monitoring to achieve optimal outcomes and is focused on chronic disease state management as part of a coordinated team.

Job Description:

EDUCATION
• Delivers education and counseling to patients on medications.
• Proactively contacts patients after hospital discharge to reconcile medications, answer questions on and new medications that were started in the hospital, evaluates patient compliance with new drug regimens, and follows-up with the patient regarding side effects of new medications
• Provides education of practice staff and team on contemporary issues related to pharmacotherapy and outcomes management including communication efforts related to mediation management.
• Supports improvement efforts related to drug intelligence – research for evidence-based medication use and education to the same.
• Supports process improvement efforts, and educates co-workers around:
◦ Medication reconciliation
◦ Medication renewals
◦ Prior authorizations


DEPARTMENT OPERATIONS
• Actively reports data analytics and cost analysis as program demands
• Participates as an active member of the Pharmacy and Therapeutics Committee Reviews health plan high risk mediation lists for improvement opportunities.
• Submits referrals to Case managers and social workers as appropriate.
• Works to improve practice standardization including policy and procedure and workflow development.
• Identifies new systems, processes, protocols and/or methods to improve practices.
• Actively contributes to the creation of cost-effective practices that ensure the best patient experience, effective resource utilization, and enhance Care Coordination outcomes
• Effectively collaborates on process improvement activities and in development of staff/ community education.
• Ensures all Care Coordination outcomes are attained (e.g, decreased readmission rates, and others as determined, etc).
• Maintains accurate, current, complete documentation using approved forms and format according to department standards, including patient assessments, plans, interventions, patient/family involvement, outside agency communications and interdisciplinary contacts.
• Complies with all requirements for mandated reporting, risk management and other medical/legal situations consistent with confidentiality policies and department standards.
• Completes all annual competency validation requirements.



EDUCATION:


Doctorate: Graduate of an accredited school of pharmacy.

Pay Range is $90.94 to $104.56 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
