Manage the overall operations of the department in accordance with current federal and state regulations and guidelines. Provides operational leadership for an integrated social services program, ensuring the psychosocial, emotional and medical needs of patients and/or a targeted population group are addressed to support optimal health and best clinical outcomes. Collaborates with the interdisciplinary team to facilitate transitions/care across the continuum, providing appropriate linkages to community resources to support care and to achieve desired clinical/treatment outcomes. Ensures cost effective utilization of resources to support the achievement and/or improvement of dashboard measures and other key strategic initiatives.Job Description:

Master's: Social Work
CERTIFICATION & LICENSURE:
LCSW-Licensed Clinical Social Worker
TYPICAL EXPERIENCE:
8 years of recent relevant experience.
SKILLS AND KNOWLEDGE:
- Working knowledge of health care delivery and case management processes in a managed care environment.
- Knowledge of laws, regulations and professional standards affecting clinical social work and case management practices in an integrated delivery system (e.g., Centers for Medicare and Medicaid Services (CMS), title 22, The Joint Commission (TJC)).
- Working knowledge of available health care and community resources appropriate for populations served.
- Knowledge of quality improvement methodologies.
- Demonstrated leadership skills with the ability to mentor, motivate and engage team.
- Demonstrates a proactive approach in identifying and addressing issues and concerns.
- Ability to bring individuals together to resolve differences/issues or to achieve a common understanding or acceptance of a solution.
- Critical thinking and analytical skills with the ability to identify and analyze trends/issues and to implement solid solutions.
- Ability to manage projects, develop plan to accomplish objectives, identify obstacles and resolve operational issues.
- Organization and planning skills to effectively delegate, manage and/or re-prioritize activities and projects to meet deadlines wihle delivery quality service and maintaining a high degree of responsiveness.

Pay Range is $73.69 to $110.54 / hourThe salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
