Clinical Pharmacist - Integrated Care Management

Position Overview:

The incumbent performs the duties of a clinical pharmacist primarily in the area of a community-based outpatient practice.  The Clinical Pharmacist will work independently and in consultation with physicians and other health care providers to ensure appropriate medication therapy for individual patients within the clinic environment.  This will be accomplished using patient interviews and counseling, reviewing the medical record and medication profile, consulting with physicians, teaching, staff education, medication use evaluation, adverse reaction monitoring and resolving medication-related problems.  The clinical pharmacist’s interventions will target this high risk population through the provision of medication reconciliation and comprehensive medication use management in focused disease states. In addition, the clinical pharmacist will also assist with care coordination, disease state education and monitoring to achieve optimal outcomes. The Clinical Pharmacist will be focused on chronic disease state management as part of the Integrated Care Management team composed of Clinical Pharmacist,  RN Case manager, LCSW and Health Care Coordinator.  The Clinical Pharmacist must be capable of using his/her professional judgment and clinical expertise in the daily solving of complex problems and the in the provision of clinical information. 

The Integrated Care Management program is patient centered and designed to support Primary Cara in improving patient health outcomes and overall well being. This program strives to strengthen a patient's ability to self-manage their conditions, access appropriate levels of care, and navigate the health care system.

  • Performs comprehensive medication reconciliation and review/education
  • Completes post-discharge medication reconciliation and review
  • Performs complex medication management
  • Optimizes medication therapy based on diagnosis, labs, tolerance/side effect concerns
  • Performs complex disease state management (lifestyle, symptom review and management, medications)
  • Provides medication consultation for providers and patients (i.e. drug information/education, drug/disease/food-herbal OTC supplement interaction)
  • Answers vaccine questions


Qualifications:

Education/Licensure:

  • Doctor of Pharmacy (Pharm D) degree from an accredited college of pharmacy or equivalent work and practice experience
  • Completion of a PGY 1 Post-graduate residency is preferred.
  • Board Certification in Pharmacotherapy is highly desirable
  • Must be licensed to practice pharmacy in California

Knowledge:

  • Must have comprehensive knowledge of pharmacotherapy
  • Intermediate level computer skills required.
  • General typing skills required

Experience:

  • Experience practicing in an ambulatory care setting a plus but not required
  • Broad acute clinical background (experience in more than one population and level of care) strongly preferred

Skills:

  • Experience with motivational interviewing a plus, but not required
  • Customer focused and patient-centered
  • Ability to multi-task
  • Comfortable with change
  • Must be able to effectively communicate with, and promote cooperation and collaboration between, multiple people including: patients and their families; physicians, nurses, social workers, etc.  Ability to seek input and receive feedback in a collaborative interdisciplinary team.
  • Must be able to develop an organized work plan in a high-volume environment with rapidly changing priorities
