The Home Health Physical Therapist evaluates, establishes treatment plan, and provides physical therapy services upon physician referral for identified patients. The physical therapist performs services in an interdisciplinary environment with Occupational Therapy, speech pathology, physicians, nursing and other health care providers. All patient services are documented appropriately and updated to justify continued intervention. Essential Job Duties Include & Are Not Limited To: Provides physical therapy, as necessary per physician order, including initial evaluations, which may include start of care PT only cases, range of motion exercises, & chart documenting & filing. Coordinates appointments. Applies rehabilitation devices, such as: hand rolls, slings, braces & sets up walkers, arranges for canes. Establishes times & performs gait training with patients, ambulates patients as directed. Maintains & documents patients' charts, restorative needs & potential. Provides input into inter-disciplinary team meetings & implements recommendations with staff, as directed. Ensures appropriate policy/procedure is followed. Completes 5-6 re-visits or a PT open and 2-3 re-visits per eight hour day including completion of all paperwork.
Education and Licensure Qualifications:
- Bachelor's degree in Physical Therapy is required
- California Physical Therapist (PT) License is required
- Basic Life Support Certifications for HealthCare Provider is required
- Valid California Driving License Required
- One year of experience in ACF or rehab department preferred
- Home health experience in physical disabilities and rehabilitation is preferred
Skills and Knowledge Qualifications:
- Applicant must have a broad range of clinical knowledge and awareness of psycho/social/physical aspects of aging
- Must complete competency skills check list during first three months of hire and annually thereafter.
